WestLake Care Community is nestled in a quiet, residential neighborhood just west of Sloan’s Lake. The community provides an innovative approach in caring for guests. Embracing the importance of relationships and Care Partnerships, WestLake focuses on the well-being of the entire Care Partner team: the resident and all those professional staff members involved in his or her care.
In addition to rehab care partners, WestLake also offers nurses and CNAs who are available 24-hours a day to meet the needs of residents. These nursing care partners provide medication administration and assistance with activities of daily living, grooming, bathing, and scheduling transportation to appointments. Additional services that are provided include comprehensive pain management which encompasses whirlpool, massage, aromatherapy, pet therapy, and heat therapy; wound care, IV therapy, and Restorative Nursing.
WestLake collaborates with short-term rehabilitation Care Partners to develop the tools and support needed in order to reach their rehabilitative goals for returning home. Care Partners work with a comprehensive team of professionals who will help reach their goals through a person-directed plan of care. The WestLake team strives to be successful in facilitating rehab Care Partners to identify and to meet their rehabilitative goals. Their rehabilitation program focuses on building relationships that promote participation in individualized therapy programs and reaching goals as soon as possible.
